Special Needs Registration

If you or a loved one have any special needs or require special care and you would like to alert Portage County Sheriff's Office, take a moment and fill out the attached form and return to: Katie Royer, Communications Supervisor at kroyer@portageco.com.
This information will be put into our CAD System to alert officers in the event they are called to your address.
It is your responsibility to verify your special needs information with the Portage County Special Needs Registry. Failure to do so may result in the distribution of outdated or unknown information to first responders.
