Please choose one of the options below to request a copy of a report:

• Email PCSORecords@portageco.com
• Fax your records request to 330-298-2110
• In-person request during normal business hours
• Send a written request to 8240 Infirmary Rd. Ravenna, OH 44266
• A charge of $.10 per page will apply. Cash, certified Check, or MO payable to: Portage County Treasurer
• Payment must be received before the release of documents. 

When making a records request,  please supply the following, along with your name and phone number if there are any questions:

• Report number (if available)
• Location of incident
• Date of Incident
• Persons Involved
